The CoP is the legal and regulatory requirements that hospitals and case management professionals must follow to be compliant in their role as discharge planners. Why Should You Attend?
In this program, we will review start with reviewing the current rules for discharge planning as outlined under the Conditions of Participation (CoP) from the Centers for Medicare and Medicaid Services. Are you compliant with these rules? We will follow this review with an overview of the final rules adopted in late 2019. These rules include requirements that will require some changes in the way you currently perform the process of discharge planning including the information provided to patients and families. We will also discuss how to assess a patient as you begin to develop your discharge plan on the day of admission. What data sets do you need to include in order to be in compliance with the (CoP)?
